Folder - yes it could be too!  I can't see any counting number wheels but wondered if it counted sheets of paper?  There seems to be an offset on the brass drum/shaft, as if it may move something once every rev or movement.  A good puzzle - thanks for the hints!

Oooooer I got it right, I wasn't even going to attempt to work this one out cos it looked and sounded so mechanical.
